We conducted a cross-sectional survey of a random sample of 1943 spouses of home-dwellers with Alzheimer’s disease (AD) to examine the prevalence of court-appointed guardians or financial powers of attorney for persons with AD, related factors and the need for information about these issues among caregiving families. The questionnaire consisted questions on variables of demographic characteristics, disability, symptoms and care needs of the person with dementia, the strain of caregiving, the use of court-appointed legal guardians or powers of attorney, as (...) well as discussions about these issues—and the need for them—with a doctor. The response rate was 77% and the mean ages of those with AD and caregivers were 80.2 and 78.2 years, respectively. The use of legal guardians was rare (4.3%), while the use of financial powers of attorney was more common (37.8%). Only 9.9% of the couples had discussed these issues with their doctor, whereas 47.9% expressed a need for it. The factors associated with the use of these legal arrangements were related to the severity of dementia, including experiencing dementia symptoms for more than 3 years, poor functioning, incontinence and behavioural symptoms. There is a clear need for information on medico-legal issues related to dementia among caregivers of AD patients. If held soon after the diagnosis, such discussions could support the autonomy of these persons in spite of AD and enable them to plan for the future as they wish. (shrink)

In this article, we present results from a literature review of intrinsic, instrumental, and relational values of nature conducted for the Intergovernmental Science-Policy Platform on Biodiversity and Ecosystem Services, as part of the Methodological Assessment of the Diverse Values and Valuations of Nature. We identify the most frequently recurring meanings in the heterogeneous use of different value types and their association with worldviews and other key concepts. From frequent uses, we determine a core meaning for each value type, which is (...) sufficiently inclusive to serve as an umbrella over different understandings in the literature and specific enough to help highlight its difference from the other types of values. Finally, we discuss convergences, overlapping areas, and fuzzy boundaries between different value types to facilitate dialogue, reduce misunderstandings, and improve the methods for valuation of nature's contributions to people, including ecosystem services, to inform policy and direct future research. © 2023 The Author(s). The concept of an "existential perspective of an ethical theory"--"its view of the world and its properties, man's nature and condition, insofar as these enter into its understanding of moral processes and moral judgments"--is sketched and used to clarify the diversity of ethical approaches. Teaching allows human culture to exist and to develop. Despite its significance, it has not been studied in depth by the cognitive neurosciences. Here we propose two hypotheses to boost the claim that teaching is a human instinct, and to expand our understanding of how teaching occurs as a dynamic bi-directional relation within the teacher-learner dyad. First, we explore how children naturally use ostensive communication when teaching; allowing them to be set in the emitter side of natural pedagogy. Then, we (...) hypothesize that the capacity to teach may precede to even have a mature metacognition and, we argue that a teacher will benefit from the interaction with her student, improving her understanding on both contents of knowledge: her own and her student’s. Meanings are construed by analogy with offices. But in contrast with similar views, offices are seen as "entities." In the final analysis all offices are reduced to that of stating facts. If the latter thesis were adequately defended the book would mark another reversal of philosophical thinking. But the author limits himself to reviving, restating, and modifying earlier positivist attempts to reduce all meaning to description.--A. P. D. M.

For this blending of philosophical analysis and empirical linguistics, however, no defense is offered, though in the course of his sketch the author registers subtle and unconventional insights into key concepts and issues in philosophy of language.--A. In refutation of Cajetan, the sixteenth century commentator who is still considered an authority on Thomas' doctrine of analogy, it is argued that "the analogy of names is, for St. Thomas, a logical intention, and in speaking of it we must observe the general rule that the logical and real orders must not be confused. St. Thomas does not see any peculiar significance of analogy for metaphysics--apart, i.e., from the significance it has for science and ordinary discourse." This controversial book professes to be a "critical account of Linguistic Philosophy and a study of Ideology." It is clearly written and abounds with entertaining aphorisms. Philosophically important objections against the Oxford School are developed in the first hundred pages. The rest is highly repetitious, and, aside from rhetorical effects and remarks on the psychology and sociology of the movement, contribute little to a "refutation" of Linguistic Philosophy. As an alternative to Linguistic Philosophy, the book only vaguely hints to a (...) "clean," "non-oblique," and "non-insinuated" form of naturalism.--A. P. D. M. (shrink)

A series of four lectures given in Athens during the author's tenure of a Fulbright Fellowship. The intention is to introduce Athenian public to three classical American philosophers, as well as to contemporary trends. The author sees interesting parallels between Emerson and the Byzantine Mystics and predicts that the interest of Americans in Ancient Greek philosophy will lead to closer studies of Byzantine philosophy. The chief defect of the book is its willingness to sacrifice content for coverage. In the course of establishing this thesis he makes some interesting suggestions on the question of ordinary language. For him this idiom is the "culture language"; it embodies the cultural experience of an age; it can be neither technical nor colloquial; it is the language of the honnête homme. Such concepts as cause, reason, consequence, condition, opportunity, and explanation are carefully investigated. The use of the causal notions in the Anglo-American tradition of tort, contract, and criminal law, as well as in continental theories, is examined in detail. The substance and development of Eddington's "scientific epistemology" are presented in close parallelism with the philosophy of Kant. The author argues that Kant was far more successful in displaying the philosophical structure of Newtonian physics than Eddington is in showing the philosophy behind quantum mechanics and relativity.--A. More than half of the book is devoted to what is almost a catalogue exposition of Western moral philosophy from Kant to Nowell-Smith. Although the author claims in the preface to have included "suitable references to Indian points of view, including the Buddhist," what references there are are usually too general, and never fully explained to be of any use to Western readers. Topics in the standard introductory logic course are all covered, with emphasis on the understanding of concepts rather than the development of skills in symbolic manipulation. Exposition is informal and rich in illustrations from reasoning in ordinary life. Exercises are also drawn from ordinary discourse, but they often call for the application of symbolic techniques. In combining accuracy and sophistication with simplicity and liveliness of style, the book is indeed a pedagogic success. --A. P. D. M.

Health care organizations are increasingly asked to show accountability about their performance. This paper proposes that accountability can best be achieved through evaluative methods that are based on evidence regarding the relationship between processes of care and expected outcomes. Root cause analysis (RCA) is used as an illustration of how a generic method of inquiring can be transformed into an ongoing monitoring, evaluation, user education and accountability strategy. The chronic worldwide lack of organs for transplantation and the continuing improvement of strategies for in situ organ preservation have led to renewed interest in elective non-therapeutic ventilation of potential organ donors. Two types of situation may be eligible for elective intensive care: patients definitely evolving towards brain death and patients suitable as controlled non-heart beating organ donors after life-supporting therapies have been assessed as futile and withdrawn. Assessment of the ethical acceptability and the risks of these strategies is essential. (...) We here offer such an ethical assessment using the four principles of medical ethics of Beauchamp and Childress applying them in their broadest sense so as to include patients and their families, their caregivers, other potential recipients of intensive care, and indeed society as a whole. The main ethical problems emerging are the definition of beneficence for the potential organ donor, the dilemma between the duty to respect a dying patient's autonomy and the duty not to harm him/her, and the possible psychological and social harm for families, caregivers other potential recipients of therapeutic intensive care, and society more generally. Caution is expressed about the ethical acceptability of elective non-therapeutic ventilation, along with some proposals for precautionary measures to be taken if it is to be implemented. (shrink)

The purpose of this study is to investigate the role of school principal trustworthiness components (i.e., ability, integrity, benevolence) in helping shape teacher trust and affective commitment within schools. Using data from 1,026 teachers in Spain and structural equation modeling (via EQS 6.3), this study establishes how a principal’s integrity and benevolence are key in determining, both directly and indirectly (via trust in the principal), teachers’ affective commitment to their school. It also reveals that the perceived ability of a principal (...) is not effective in generating trust or affective commitment to the school in teachers. As a contribution to the literature, these findings reveal that the ethical trustworthiness (benevolence, integrity) of principals is key in engendering trust and affective commitment to schools. Thus, to shape work environments that are high in trust and commitment, principals should focus on behaving in ways that make their integrity and benevolence manifest to their teachers. (shrink)

Introduction: The decision of “do not attempt resuscitation” in the event of cardiopulmonary arrest is usually made when the patients are critically ill and cannot make an informed choice. Although, various professional bodies have published guidelines, little is know about the patients’ own views regarding DNAR discussion.Aim: The aim of this study was to determine patients’ attitudes regarding discussing DNAR before they are critically ill.Methods: A prospective study was performed in a general out patients department. A questionnaire was distributed to (...) consecutive outpatients along with an explanatory leaflet in the adult outpatient clinic.Results: 364 patients completed the questionnaire . 90% of respondents wanted all patients to be asked regarding DNAR decision at some point during a hospital admission. The majority would not find a DNAR discussion distressing. Only 10% would find it upsetting, however, 48% of these still wanted a discussion. 37% of respondents wanted to discuss DNAR decisions on admission; 32% in outpatients; 17% at consent for surgery, 14% when they are critically ill. 87% of respondents would not object to their relatives being involved in making decisions about their resuscitation status. However, only 12% of the subjects in the study had been involved in discussing the resuscitation status of a relative and 21% would not be comfortable to discuss a relative’s resuscitation status. Although 33% of patients preferred their resuscitation status to simply be documented within their clinical notes, 77% wanted it to be more easily accessible.Conclusions: This study suggests that contrary to current practice most patients want to discuss their DNAR status prior to becoming critically ill. This includes half of the small number that find it distressing to discuss. Although most patients are comfortable with relatives being involved in discussing DNAR, a significant proportion do not want their relatives to be asked. Furthermore, once a decision has been made, the majority of patients want it to be more accessible than current practice allows. (shrink)
